Notice to Insureds/Applicants in Storm Damaged Areas 

Central States Health & Life Co. of Omaha is aware that storm-damaged areas of the U.S. may be experiencing mail delivery delays.  If you recently received correspondence from us requiring you to respond with claim information or a premium notice within a specified time period or by a particular date, and you find that you are unable to do so for storm related reasons, please notify us and we will work with you on extending your response deadline.  You may contact our office at 800-826-6587 with any questions or concerns. 

NOTE:  Illinois consumers may contact the Illinois Division of Insurance to file a complaint regarding any disaster related dispute or issue.  The Division’s toll free number is 1-866-445-5364 or you may file a complaint online at http://www.idfpr.com/doi/Complaints/Compliants.asp.
